I don’t know VLC’s shuffle algorithm, but any random sequence includes portions that don’t feel random at all. Clearly VLC doesn’t maintain a list of songs it has already played so as to avoid this effect. Winamp/PC let you shuffle a playlist and play the result sequentially, which prevented this effect. In a universe of 40 songs, repetition is noticeable; with thousands of songs, it’s rare (but not nonexistent).
Regarding your last question, if the crossed-arrows logo is still displayed in orange, you have not left shuffle mode. I think skipping a song does not make the problem any more or less likely.
