Save recently played items is enabled.However, as I said, it doesn’t always save the position. Something I forgot to mention is that I usually access mp4s from playlists.
This has been happening for months to me, too.
And I don’t access files from playlists.
It happens only a few times that VLC resumes playback from where I stopped. 
On top of this, I uninstalled VLC, rebooted, cleaned the registry with CCleaner, rebooted, reinstalled VLC, set the right preferences but the problem still persists.
